Welcome to the Blue Ridge Plaza Animal Clinic! All new clients scheduled to see the veterinary must complete a NEW CLIENT FORM. This permanent profile helps us understand your pet's background, personality, and special needs so we can make your pet's visit as comfortable as possible. 

Vaccine Protocols

In order for us to provide the highest level of medical safety to our guests when boarding or scheduling a pet groom, we require proof of current vaccinations, by a licensed veterinarian. Depending on your city's local ordinances you may only be allowed to obtain a 1 year vaccination. Please bring your pet's vaccination record prior to your appointment or during your first visit. If you do not have a copy, please contact us 48 hours before your scheduled visit, so we can have your records faxed to us.

BOARDING DOGS: 

Rabies: 1 year or 3 year*

Bordetella/ Kennel Cough: 1 year 

Distemper: 1 year

BOARDING CATS:

Rabies: 1 year or 3 year*

FVRCP: 1 year or 3 year

GROOM DOGS:

Rabies: 1 year or 3 year*

GROOM CATS:

Rabies: 1 year or 3 year*
